IMMIGRATION EXAM
The Immigration and Naturalization Services (INS) requires that most applicants seeking adjustment of status are to have a medical examination. The medical examination must be conducted by an approved physician designated by the INS as a Civil Surgeon. The examination must be documented using form I-693.
In addition to the examination, the civil surgeon is responsible for ensuring that all the required components include specific vaccinations which include, but are not limited to: varicella, mumps, measles, rubella, polio, tetanus-diphtheria (Td)/ tetanus-diphtheria-pertussis (Tdap), influenza, hepatitis B and covid-19 as recommended by the Advisory Committee for Immunization Practices (ACIP).
After the Civil Surgeon has completed the examination, Form I-693 and the supplemental Form, they shall be placed in a sealed envelope and given to the applicant. DO NOT OPEN THE SEALED ENVELOPE.
